Job Summary:
Primary functions are to administer skilled nursing care for Senior Adults in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency and assumes the responsibility of coordination of care.

  • Provide skilled nursing care to patients on a recurring basis in their place of residence.
  • Provide services in accordance with the patient's care plan, in coordination with other members of the health care team, and in compliance with all regulatory guidelines.
  • Evaluate the patient, family members, and home environment to determine required services; continually assess the patient's needs.
  • Review and provide supervision for each patient's care plan; stay informed of services, diagnoses, medications, procedures, goals and progress.
  • Teach, supervise and counsel the patient and family members regarding the nursing care needs.
  • Coordinate and supervise care provided by LVN and paraprofessionals assigned to the patient and assure continuity of quality patient care.
  • Consult with the attending physician regarding any changes to the plan of treatment under direct supervision of the Director of Nursing.
  • Coordinate transfer and discharge of patient from agency services as indicated by the physician.
  • Perform on-call responsibilities and provide on-call services to patients/families as assigned.
  • Submit all patient notes and/or visit records on a timely basis.
  • Participate in interdisciplinary team conferences.
  • Attend on-the-job and in-service training.

  • Graduate of an accredited School of Nursing
  • Current Texas State license as a Registered Nurse
  • Current Texas Driver License and Auto Insurance
  • Minimum 1 year RN Home Health nurse REQUIRED

  • Works in patient's home in various conditions
  • Assess home health patients to identify the physical, psycho social and environmental needs
  • Requires working under some stressful conditions to meet deadlines and patient needs
  • Ability to work flexible schedule.
  • Ability to travel locally
  • PRN emergency call
  • Promote Agency philosophy and administrative policies
